What is Personal Information (PI) and why do I collect it?
PI is information that identifies an individual such as names, addresses, email addresses, and phone numbers. PI is obtained in many ways including by website contact form, patient intake form, email, phone, from media and publications, website cookies and third parties.
I collect your PI for the primary purpose of providing my services to you and for marketing. I may also use your PI for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. Where appropriate and possible, I will explain to you why I’m collecting personal information and how I plan to use it. You may unsubscribe from my mailing/marketing lists at any time.

With any emergency contacts or care providers in the event you cannot act on your own behalf due to serious illness

Security of Personal Information
Your PI is stored in a manner that reasonably protects it from misuse, loss and from unauthorized access, modification or disclosure. When your PI is no longer needed for the purpose for which it was obtained, I will take reasonable steps to destroy or permanently de-identify it. However, most PI is or will be stored in client files which will be kept for a minimum of 7 years. If you wish to access your PI, please contact me in writing.
Maintaining the Quality of your Personal Information
I will take reasonable steps to make sure that your PI is accurate, complete and up-to-date. If you find that the information I have is not up to date or is inaccurate, please advise me as soon as practicable so I can update my records and ensure I can continue to provide quality services to you.
